Thank you for helping Children's Wisconsin provide the best and safest care to kids and families.

As part of the Children's E-Advisory Team, you will give us your feedback using online surveys. This will help us give better service and care and learn what matters most to you. To join this team, please fill out this form. You agree to the following:

• I am 18 years of age or older.
• I will not receive any type of payment for being part of this team. 
• I will not share any information from the surveys. 
• I will answer a minimum of 6 surveys per year.
• I may choose not to complete a specific survey at any time.
• I will use the system as it is intended.
• The program is not a tool for complaints.
• I may stop being a part of the E-Advisory team at any time.

What you can expect from us:

• Surveys will be sent by email. You will not get more than two surveys in a month.
• You may need to check boxes, rank items, or give feedback. 
• You may get a reminder email to complete the survey.  
• Survey results will not be shared until all responses are combined. No individual answers will be shared.
• Some incentives for doing a survey may be offered.

Confidentiality Statement: 

The term 'confidential information' means any information or material which belongs to Children's Hospital and Health System. You agree not to share any of the materials or information that would be viewed through the surveys. The information collected on this application will be kept confidential and will be used only for system and process improvement work.
